TVGOS Host channel submission form

I am always looking to improve the my unofficial database of TVGOS/Gemstar host channels. If you know your TVGOS host channel for your area, have confirmed it via the service menus as described in Section 2.1.8 of the FAQ/Resource guide, and have confirmed that your ZIP code isn't already covered in the list, please fill out the info below so I can add it to the list.
